[UX] Move the confirmation dialog for enabling/uninstalling modules to a modal.

Part of the #3769 meta-issue.

As part of improving the UX for #541, we should move the confirmation dialog to a modal.

You can easily check phpinfo() by going to the Status Report (admin/reports/status) and clicking the link against the PHP item:
