Rule based on elapsed "Last Access Time" for all users
This kept happening to me. I finally fixed it by going into the admin bar settings and turning off the "Adjust top margin" option. I suspect the issues might have been caused by my CSS...
Admin menu disappeared after updating to core 1.29.3
Still haven't figured this out, but seems, but I can't leave or edit comments when logged in as administrator. I re-edited and cleaned up the allowed html tags filter, rebuilt permissions...
