Is it possible to add a screenshot to a contrib theme or module. I thought we had done work on this, but I'm not finding documentation on it or even confirmation that it is possible.

So maybe it's not even related to other modules, but some... cache problem somewhere? Be it browser or server-side? A Heisenbug.
It remains a complete mystery. I started enabling other modules on my clean install and boom - off it went again. Of course, removing those modules didn’t make any difference so I couldn’t...

I don't think, there's any official documentation or policy regarding screenshots.
Which means: you can do what you want. :-)
Probably a custom directory and then add the screenshots with full url to your README?
