As an admin, I'm logged into my site and the title block keeps disappearing. If I clear the cache it comes back, but if I refresh the page it disappears again.
Any ideas?

Turns out there is a bug that causes mysterious problems if you remove the Main Content Block from a template. There is a warning on the block to this effect, but somehow I didn't notice (or was distracted).
The problem for me was masked by the fact that it did not appear while I had the setting to "Rebuild the theme registry on every page load" checked (devel module). However, as soon as I turned this off, the problem showed up.
